Damaged Shade Claim

To insure the quickest turnaround to replace shades that arrived damaged, please fill out this form. As soon as this form is submitted, we can begin the claim process. Damaged shade claims must be made within 5 days of delivery to qualify for replacement.

Photos are required by the shipping company to process a claim.  If you are unable to upload a photo using this form, you must email, fax or mail photos to document the damage.  A damage claim is not considered complete entered until we receive photos.

Every damage claim requires a minimum of one photo showing each damaged shade. If the box is damaged, please also send a photo of the damaged box. Damage claims submitted without photos are not processed. Sorry, no exceptions.
